At Traditions Health, our team of caring professionals understands the scope of having a military background and it is our privilege to honor that service with individualized care plans. Veterans often face challenges related to their service, including emotional and physical trauma, and our expert hospice team works to honor each veteran’s needs through their customized care.
Every veteran has a story marked by sacrifice, resilience, and courage — and for that we are profoundly grateful. Beyond gratitude, these heroes deserve the highest level of care and support as they face an end-of-life illness. Traditions Health aims to recognize the unique needs of veterans and their families. Here’s how some of our branches honor our veteran patients:
We Honor Veterans (WHV) was developed by the National Alliance for Care at Home in collaboration with the Department of Veterans Affairs (VA). The WHV program’s impact is far-reaching: It provides veterans with comfort, understanding, and a better quality of life at the end of their lives. It’s also a source of comfort for families, knowing their loved ones will receive the best customized care possible.
Many Traditions Health locations work in cooperation with We Honor Veterans to ensure we provide those who have served our country with highly skilled and compassionate end-of-life care.
We are passionate about giving veterans high-quality, compassionate care at the end of their lives and it is our privilege to honor that service with individualized care plans. Under the direction of a physician, the Traditions Health team offers a caring atmosphere that addresses the emotional, and physical well-being of our patients.
In addition to the core members listed and depending on each veteran’s needs, a patient's individualized care team may also include a hospice health aide, bereavement support services, and volunteers.
Through our partnership with WHV, we ensure our team has the knowledge and skills to provide the best possible care for our veterans. However, for us to continue providing this level of care, we also rely on volunteers who come from varied military backgrounds, have worked closely with someone who served in the military, or have served in the military themselves. Volunteers often support through different activities, including the following:
Traditions Health continues to expand across the country offering access to quality care to those in need. As we expand, we remain committed to the same values on which we were founded — prioritizing patient-focused, compassionate care, while building strong relationships in the communities we serve.
